What is Intermediate Care?

Intermediate or after hospital care services are those provided to elderly people at home (or in a residential setting) for a short period of time. They help people with complex care needs to recuperate, recover or convalesce after an illness or hospital admission and to maximise their independence.

The service aims to prevent unnecessary hospital admission and to provide rehabilitation in a residential setting. This can help older people to regain or maximise their daily living skills, improve their confidence and help them to remain in their home. Community rehabilitation of this nature also supports elderly people who are recovering from an illness through a period of convalescence, and it usually last for no more than 6 weeks.
